Introduction
Medical report is a document prepared by a registered medical practitioner or registered dental practitioner who served the Government of Malaysia (Fees Act 1951, Fees (Medical) Order 1982) on health problems and treatments received by a patient in a hospital or any health facilities based on patient’s medical record.

Verification of Medical Report
Application for a verification of medical report is a mechanism to ensure that the information or data provided by a registered medical practitioner about health problems and treatments received by a patient in a hospital or any other health facility based on the patient’s medical record is true and valid. It is also to prevent some people from taking advantage of manipulating a copy of medical report for specific purposes such as fraud information and so on.
Medical report is a document that can involve legal implications, the verification of the medical report must be made by identified registered medical practitioners, head of relevant department or director of the hospital in consultation with medical reports provided.
Application for verification of medical report can be made by mail or drop in to the Medical Record Department / Unit of the hospital where patients get the medical report. The application will only be processed once the complete application is submitted in writing along with a copy of the medical report issued.
For example, Employees Provident Fund (EPF) often dealt with verification of medical report for withdrawal application under the scheme of compensation using the format validity of the medical disability report issued by EPF.
Apart from EPF, there are also application from the insurance companies for the purpose of insurance claims management.
To date, there are no charges, payment imposed for application for the verification of medical reports.
