Medical Report Preparation In Ministry of Health Facilities

Definition

Medical Report is a document prepared by a Registered Medical or Dental Practitioner who is employed by the Government of Malaysia (Fee Act 1951, Fee (Medical) in 1982) on health problems and treatment to any patient in hospital or any health facility based on the patient’s medical history (Medical Report Guideline of the Malaysian Medical Council, January 2007).

Application for a medical report can be made by:

  1. Applying directly to the Medical Records Department in the MOH hospitals using the format KKM/LP/1/2008 (Appendix 1), which is also available at the department counter. 
  2. Writing in to the hospital director.

Documents required for the preparation of   a medical report are determined by the category of applicants (Refer Table 1).

Charges are in accordance to fee act 1951 schedule H part 4

Brief report / opinion prepared by  Medical Officers ………………….. RM40
Brief report / opinion prepared by Specialist              ………………….. RM80
Comprehensive report  prepared by  Specialist          ………………….. RM200 –  RM1000

  – depending on the complexity of reports prepared  (Refer to Table 2).

Charge  for a  foreigner is  twice the charge for a citizen.

Preparation period of the medical report:

Medical Report shall be prepared within the specified  period, initiated from the date of receipt of a complete application:

State Hospital……………………..4 weeks
Specialist Hospital………………2 weeks
Non Specialist Hospital ………2 weeks
